NACR Patient Opt Out

Request to opt-out from the National Audit of Cardiac Rehabilitation (for patients in England only)

The NHS National Audit of Cardiac Rehabilitation (NACR) uses information from cardiac rehabilitation (CR) programmes to measure the quality of CR care and services and help improve service provision for patients.

NACR has been granted exemption by the Confidentiality Advisory Group (part of the Health Research Authority) from the National Data Opt-out** (NDO). Therefore, even if you have registered for NDO you will still need to opt-out from NACR to prevent your health records being used by the audit.

If you do not want your information to be used in NACR please complete this form and email it to the NHS England Service Desk at: ssd.nationalservicedesk@nhs.net with the subject heading of “Patient Opt Out: NACR”.  Alternatively, you can call them on 0300 303 5035.

NB: The Service Desk are unable to accept opt-out forms by post.

Remember, choosing not to be in NACR will not affect your care. You can opt back in at any time.
