April 2026 NACR Quarterly Report on Mode of Delivery (rerun July 2026)

Method using July 2026 data download of Oct-Dec 2025 data; patient records were included if they had started Core1 Rehab within the time period and there was sufficient data - more than 10 starting core rehab - within the named programme. This report assesses recording and provision of CR delivery modes. A menu-based approach driven by patient choice is a key recommendation in the BACPR 2023 Standards and Core Components and an important audit metric.

Results Of 183 programmes with starters, 176 had sufficient data for analysis. Of these 176, 3% had no mode recorded at all, and in total 17% of patients had no mode recorded. Most programmes (150) offered multiple delivery modes, while 20 used a single mode, predominantly Group-based. Over 80% of programmes showed patients receiving either Group-based or Home-based alone, whereas 73% of programmes had patients with Hybrid delivery (both Group- and Home-based). This shows a diverse profile of modes in many services. Delivery patterns for Oct–Dec 2025 show 39% of patients received Group-based CR, 31% Home-based and 27% Hybrid. Hybrid delivery has increased since the first running of this quarterly report topic in 2023, where it was 23%, while Home-based delivery has declined from 40%. A new measure identifies digital delivery (Web Other, App Other or Activate Your Heart). Overall, 6% of patients received a digital component, although 97 of 175 programmes recorded none. Additional data collection is planned to support enhanced reporting in line with the NICE Early Value Assessment of digital CR platforms.

(1 Throughout this report ‘Core’ refers to Core or Phase 3 records)

(* Patient received group and home-based CR )

(** This mode is where there is an assessment 1 and structured programme initiated, however, patients lead their own programme and are supported, limitedly, by support/contact with the CR team. It also requires an expectation of completion of Assessment 2 )
